Medical History

Provided here are the documents that must be filled out about your personal medical history and your insurance information. By downloading and filling these out at home and coming in prepared, you will not have to wait longer in the office during your first visit.

Your first visit to our New York City dental practice will always be a complementary problem focused consultation.  This visit allows you to meet our team and our caring dentists. If an X-ray is needed that is also complementary.  We feel it is very important for the doctor and the patient to share the vision of what treatment, if any, must be completed.  Our use of the latest diagnostic instruments and photographs will allow us to share our knowledge with you.  Any treatment would be rendered at this visit only IF, YOU choose to proceed. Or you may choose to come back to our Manhattan dental office for another visit.
